Bakersfield, CA Job Summary: We are seeking a dedicated Primary
Care Physician to join our team at Bakersfield, CA . The successful
candidate will provide primary medical care, focusing on
comprehensive health assessments, care plan development, direct
patient care, and ongoing evaluation in collaboration with the
interdisciplinary team. As a Primary Care Physician , you will
ensure high-quality care for participants, including the frail
elderly, in an interdisciplinary environment. You will have the
opportunity to make independent medical decisions while working
alongside other healthcare professionals. Key Responsibilities:
Assessment: Participate as a member of the interdisciplinary team
(IDT) during initial, semi-annual, unscheduled, and annual
assessments. Provide ongoing assessments , develop care plans, and
implement them in collaboration with the team. Obtain comprehensive
health histories and systematically record findings. Perform
physical examinations , order and evaluate diagnostic tests, and
record results. Identify medical, social, cultural, and financial
problems affecting participants. Conduct functional and mental
status evaluations using standardized procedures. Triage status
changes and facilitate appropriate management of health concerns.
Treatment: Provide preventative healthcare and health promotion for
participants. Treat and manage acute and chronic illnesses ,
including medication regulation. Facilitate specialist
consultations as required. Educate participants and families on
health maintenance and chronic conditions. Provide inpatient
attending coverage when participants are hospitalized or in nursing
facilities. Supervise nurse practitioners or physician assistants
in collaborative care settings. Evaluation: Serve as a source of
medical knowledge for the interdisciplinary team, especially in
geriatric care. Evaluate and adjust the participant's care plan to
ensure effectiveness. Maintain accurate documentation of care
provided and coordinate services. Participate in regular
interdisciplinary team meetings to review and monitor health
statuses. Consultation & Administrative Duties: Integrate actively
into the interdisciplinary team , contributing medical knowledge
and insights. Serve on committees and contribute to the development
of policies and procedures. Prepare and submit reports on services
provided, as required. Stay up to date with healthcare trends
through relevant conferences and courses. Complete charting and
paperwork in a timely and accurate manner. Comply with company
policies, procedures, and protocols , including completing time
sheets and attending staff meetings. Working Conditions and
Physical Demands: Ability to access all areas of the facility
throughout the workday. Ability to lift up to 35 pounds
occasionally, 15 pounds frequently, and 7 pounds constantly.
Requires frequent sitting, standing, walking, and repetitive
movements. Ability to communicate clearly and effectively in
English. Work environment is indoor and well-maintained
(temperature-controlled). Qualifications: Experience:
Board-certified/eligible in Internal Medicine or Family Practice .
Advanced certification in geriatrics and board certification for
Added Qualification in Geriatric Medicine preferred. Minimum of 1
year of experience working with frail or elderly populations .
Knowledge, Skills, and Abilities: Deep interest in the care of
frail older adults , with a solid understanding of their physical,
mental, and social needs. Proficient in chronic disease management
and physical assessments for geriatric populations. Ability to work
in a creative, alternate healthcare setting . Strong organizational
skills , with the ability to manage shifting priorities according
to participant needs. Effective communication skills , both oral
and written. Collaborative team player with the ability to work
with diverse healthcare professionals. Education, Licensure, and
Certification: Licensed to practice medicine in the state of
California. Current DEA registration and ability to obtain staff
privileges at affiliated hospitals. CPR/BLS certification required.
Must be medically cleared for communicable diseases and have
up-to-date immunizations before engaging in direct patient contact.
