Job Title: Registered Nurse - Case Management Profession: RN Specialty: Case Management Duration: 12 months Shift: Monday - Friday Hours per Shift: 08:00 - 16:30 Experience: Minimum of 2 years recent RN Case Management and discharge planning experience in a hospital setting License: Must hold a California RN license Certifications: BLS certification (must be through the American Heart Association) Must-Have: Must provide documentation of BLS certification. Must provide documentation of California RN license. Description: Coordinates management of care and ensures optimal utilization of resources. Ensures service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment and care planning.
